Rates
$80 to $120 per 45-55-minute session depending on whether you are a private pay person or if there is a negotiated contract price with your insurance company.
I do accept MEDICARE but not MEDICAID. I also accept Blue Cross Blue Shield, Cigna, Tricare, Humana LifeSynch, HMSA, Web TPA, Beacon and others. I am an Out Of Network Provider for Aetna and I accept Aetna from Lee County School System employees with a very small co-pay.
This is not a complete list. There are many variations in people's insurance. The thing to do is to call them before you come in so there are no surprises.
Insurance
Services may be covered in full or in part by your health insurance or employee benefit plan. If you use insurance you are responsible for any co-pays, deductibles or co-insurance that your plan requires. A credit or debit card is required to be on file for these charges. Ultimately, you are responsible for any charges not paid by your insurance so p lease check your coverage carefully by asking the following questions:
- Do I have outpatient mental health benefits?
- Do I have a co-pay?
- Do I have co-insurance?
- What is my deductible and how much has been met?
- How many sessions per year does my insurance cover?
- Is pre-approval required?
- Is there a limit on number of sessions?
- What is the coverage for out of network providers?
Payment
Cash, check and all major credit cards accepted for payment.
Cancellation Policy
If you do not show up for your scheduled therapy appointment, and you have not notified me at least 24 hours in advance, you will be required to pay a $50 missed appointment fee.
